Washington Department of Health: State revokes, suspends licenses, certifications and registrations of health care providers



OLYMPIA, Washington, August 3 — The Washington State Department of Health issued the following press release:

the Washington State Department of Health has revoked or suspended the licenses, certifications or registrations of the following healthcare providers in our state. The department also immediately suspended the credentials of people who were banned from practicing in other states.

The Ministry’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to establish accreditation standards for more than 80 health professions (e.g., dentists, nurses, counselors ). Information on disciplinary measures taken against doctors and medical assistants is available on the website Washington Medical Commission (WMC) website. Questions about WMC’s disciplinary action can be directed to [email protected].

Information on health care providers can be found on the agency’s website. Click on “Search for a health care provider license” in the “How do I?” Section. »Part of the health department home page (doh.wa.gov). The site includes information about a healthcare provider‘s license status, the expiration and renewal date of their credentials, disciplinary action, and copies of legal documents issued after. July 1998. This information is also available by calling 360-236-4700. Consumers who believe a healthcare provider has acted unprofessionally are also encouraged to call and report their complaint.

Grant County

In June 2021 the Pharmacy Commission entered an order agreed with the pharmacy assistant Sarah Estelle Hicks (VB60960718) and has suspended his license indefinitely. Hicks admitted to her employer that she diverted 246 hydrocodone, oxycodone and tramadol tablets for her personal use over a three month period.

King County

In June 2021 the Chiropractic Commission indefinitely suspended the chiropractor’s license James robert carpenter (CH00034218). The commission received a complaint alleging that Carpenter practiced without a license and submitted claims to an insurance company. The commission requested all patient records and a response to the charges and Carpenter did not provide any of the requested records. He also responded to the letter saying that a car crashed into his office and destroyed all of his patients’ records.

In May 2021 health secretary suspended Licensed Practical Nurse and Home Help licenses Rachelle rose smith (NA60938557, HM60762526). Smith and a patient first developed a friendship, then a romantic relationship. Smith became the patient’s enduring power of attorney and they moved in together.

Pierce County

In May 2021 the counselors program revoked the licenses of trainees in substance use disorders and counselors affiliated with the agency James michael counts (CG60840987, CO60812938). Counts had sex with a client.

In June 2021 the health secretary suspended the licensed practical nurse’s license Jesse jay hill (NA60540266). Hill was convicted of third degree assault, as well as two counts of violating a protection order – domestic violence. Hill did not respond to a health care investigator’s investigation.

Skagit County

In June 2021 the health secretary has suspended indefinitely the licenses of professional trainee in substance use disorders and of counselor affiliated with the agency Martha Elisabeth Thompson (CP60879813, CO60445557). Thompson did not respond to a health department three requests for an investigator’s response to a complaint against her.

Snohomish County

In May 2021 examination Psychology counseling suspended indefinitely the license of psychologist Michael L. Olson (PY00001263). Olson practiced below standards of care by becoming excessively involved with a client, assuming and continuing a post-therapy gatekeeper role, which blurred practitioner-client lines and put the client at risk of harm. Olson would not have kept adequate records for the client.

Spokane County

In June 2021 the Home Help Program suspended the certified home help permit for an indefinite period Ryan anthony duncan (HM60901877). Duncan admitted to having recently used methamphetamine.

Thurston County

In June 2021 the Physiotherapy Council has indefinitely suspended the physiotherapist assistant license Christophe Joseph Edwards (P160409046). Edwards broke an agreed order.

Walla Walla County

In June 2021 the Substance Use Disorders Professional Program has entered into an agreed order and suspended the Substance Use Disorders Professional license Shelly ann hadaller (CP60644176) for at least five years. Hadaller began a romantic relationship with a patient and later married the patient.

Yakima County

In June 2021 the Pharmacy Commission suspended the pharmacy assistant license Angeles torres (VB60955748). the Washington State Department of Health and Social Services determined that Torres had neglected a vulnerable adult and put Torres on his long-term care abuse and neglect registry. This prohibits Torres from being employed to care for or access vulnerable adults without supervision.

Out of state

California: In June 2021 the Nursing Commission indefinitely suspended the RN license Ashley Verona Stone (RN60279309). On five occasions, Stone allegedly removed ten syringes and three tablets of controlled substances (fentanyl, hydromorphone) from a PYXIS device and failed to document whether any part of the drug had been administered to patients, wasted or returned.

Oregon: In June 2021 the secretary of health mental health advisor in charge of James randy webb (LH60185615) with unprofessional conduct and immediately suspended his license. The fees say the Arizona Board of Behavioral Health Examiners revoked Webb’s license to practice as a counselor on the grounds that he had violated the laws governing the practice of counseling in Arizona.
